BSNL Valid Documents as Proof of Identity & proof of Address

For applying any New Telecom service in BSNL, it has made compulsory to submit the valid documents such as proof of identity and proof of address along with the application form.
Recently DoT (Department of Telecommunications) has given instructions on verification of New Customers(especially in Mobile services-Prepaid/Postpaid) which shall be followed
by the Telecom Service Providers.

A Passport Size Photograph of the Customer should be pasted on the Customer Acquisition Form(CAF) and
documents as Proof of Identity and Proof of Address of the customer should be attached with the CAF
while applying for new telecom services. The list of valid documents approved by BSNL are as follows

Valid Documents as Proof of Identity:

    Aadhar Card
    Passport with validity
    Arms License with validity
    Driving License with validity
    Voter ID with date of issue
    Ration card having photo with date of issue(period of validity)
    CGHS/ECHS card having photo with date of issue
    Certificate with photo issued by Gazetted Officer/MLA/MP on their letter head with date
    Certificate with photo issued by recognized educational institutes for students only
    Certificate with photo issued by village heads for rural area customers only
    PAN card with date of issue
    Post office/scheduled bank password having photo along with 3 recent months transactions
    Students Photo ID card issued by government recognized educational institutions with validity
    Pensioner card having photo
    Photo ID card issued of Central govt./State govt./PSU only with validity/date of issue

    Photo ID card issued of Central govt./State govt./PSU only with validity/date of issue
